Insurance and Payment Policies

We accept the following insurance plans:
  • Aetna
  • ASR Health Benefits
  • Blue Care Network
  • Blue Cross Blue Shield
  • Cofinity
  • McLaren Health Plan
  • Priority Health

Other insurance plans may be accepted if the policy includes “Out of Network” benefits.

VERIFY ELIGIBILITY & BENEFITS

Please contact your insurance company to verify your eligibility and benefits. Insurance companies have the right to change copays, coinsurances, and deductibles at any time, so it is wise to be aware of your financial responsibility.

Payments and Fees

Credit Card on File                                                   

A credit or debit card is required to be held on file. No exceptions. You may use this form of payment for your financial responsibility at the end of each appointment or use another credit/debit card, cash or check.

Email and Phone Response Fees

Communication outside of our sessions is typically to schedule or reschedule appointments. I do understand there are circumstances outside of our sessions that may require additional time. Please note the following fee schedule for the circumstances that extend past 10 minutes on the phone or via email and be aware that insurance does not cover these fees:

  • 11-20 minutes $30.00
  • 21-40 minutes $60.00
  • 41-60 minutes $90.00
Cancellations/Missed Appointments/Late Appointments

Once your appointment is scheduled, that time is reserved for you.

Any cancellation must occur at least 24 hours in advance of your scheduled appointment to avoid a cancellation fee of $50 which will be charged to your card on file. Should you miss your appointment and not contact 8th Street Counseling within 24 hours of your scheduled appointment, your card on file will be charged $50. If you arrive 30 minutes late from your scheduled time, your appointment time is forfeited and your credit card will be charged $50.

There are two exceptions to this policy:

1. Severe Weather – If you do not feel safe to drive, please contact 8th Street Counseling as soon as possible.
2. Medical Emergency – If you or a family member has one, please contact 8th Street Counseling as soon as you can.
